The role

This is a fantastic opportunity to be a part of shaping integrated care, leading teams and working with partners to improve outcomes and experiences for the people of Warrington.

We are looking for a values led, dynamic, outcomes driven and compassionate individual to join the service at a time of significant opportunity. We are well on our journey to re-set Adult Social Care with Intermediate Care playing a pivotal role in preventing, reducing and delaying the need for long term care.

You will work as part of a team with the Service Managers for the Integrated Hospital Discharge Team and Intermediate Care Bed bases. Health and social care staff work as integrated teams across the portfolio supporting people to live well and independently in the community. Close working with our hospital and other community colleagues is central to our shared philosophy in improving outcomes for people in Warrington.

Working as a member of a forward thinking, supportive and ‘can do’ senior management team, you will have overall responsibility for the Intermediate Care at Home, Assisted Living, Urgent Community Response, Telecare and Care Call services.

As an individual you will have previous significant multi-disciplinary operational experience at a managerial level, excellent interpersonal skills, able to work in genuine collaboration with others, a strong person-centred value base and be able to deliver responsive service changes that result in high quality cost effective social and clinical outcomes.

Further Information

The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
