The role

Are you an experienced social worker looking to take the next step in your career within an integrated team?

We have an exciting opportunity for a Senior Practitioner to join our Integrated Intermediate Tier Services. You will work as part of the Transfer of Care Hub and the Intermediate Care Bed-Based Team, taking on supervisory responsibilities within a truly integrated health and social care system.

This is a full-time position (37 hours per week).

In this role, you will:

• Support safe and effective hospital discharges.
• Work within a system that promotes recovery in community or home settings where appropriate.
• Help coordinate a range of support options, including:
• Signposting to voluntary and third sector services
• Rehabilitation at home or in bed-based settings
• Care packages and home adaptations
• Long-term care arrangements where required

What's needed?

As a Senior Practitioner, you will:

• Provide supervision, mentoring, and support to team members.
• Work collaboratively with health professionals, social care colleagues, carers, families, and partner organisations.
• Take a solution-focused approach, ensuring individuals’ needs are at the centre of all decisions.
• Operate within the Care Act framework, supporting assessments, planning, and decision-making.
• Effectively manage risk in partnership with others.
• Build and maintain strong working relationships across hospital and community services.

• An experienced and confident social worker
• Passionate about supporting people to live independently
• A strong team player with excellent communication and leadership skills
• Committed to continuous improvement and high-quality service delivery
