About us

An exciting opportunity has arisen for a Grade 9 Senior Practitioner post within Warrington's Adult Community Social Work, Locality Team.

We are working in partnership across the town to shape a health and social care system to be proud of, adopting a strengths based approach to the delivery of high quality care and support.

The Localities Teams provide an assessment & care management service working with adults in the community living with a range of long-term and complex health and social care needs including:-

  • All adults living with dementia
  • Older people living with significant hearing and/or visual impairments
  • Older people living with frailty
  • Older people who misuse alcohol or drugs
  • Older people living with other debilitating health conditions

The role

As senior practitioner you will have a mixed role of care assessment as well as supporting the Team Manager to manage social work practitioners to deliver a safe and effective service for Warrington residents in need of care and support with a focus on the following key areas:-

  • Supporting Team Manager in leading the team
  • Undertaking case audits
  • Authorisations
  • Case allocations
  • Complex case discussions
  • Care Act assessments
  • Safeguarding enquiries.
  • Legal literacy
  • Supervisions and mentoring

As a valued member of the management team you will be supported by team manager with 4 weekly supervisions. You will work within the legislation of the Care Act 2014, Mental Capacity Act 2005, the Mental Health Act 1983 (amended 2007) and Human Rights Act 1998 to deliver and support your team to deliver personalised assessment, support planning and reviews in addition to safeguarding enquiries and court of protection work.

What's needed?

Applicants must meet the essential criteria set out in the person specification. You must hold a current active registration with Social Work England and a full UK driving licence. The ability to communicate at ease with members of the public and colleagues, providing clear information and advice is essential for the post. DBS clearance is required.

The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.

DBS clearance is required.
