An exciting opportunity has arisen for Grade 7/8 Adult Social Workers to join Warrington Borough Council's Localities Service.

We are working in partnership across the town to shape a health and social care system to be proud of, one that is fit for the future and built to last. We are looking for dedicated and motivated social workers who share our commitment to delivering high-quality, strengths-based and person-centred support to adults across Warrington.

The Localities Service consists of four geographical teams covering South, West, East and Central Warrington. Each team is supported by a Team Manager, Senior Practitioner, Social Workers, Community Care Workers and dedicated administrative support. As part of our supportive and experienced workforce, you will work alongside colleagues and partner agencies to help adults maintain their independence, wellbeing and quality of life.

We are committed to investing in our workforce and offer a wide range of learning and development opportunities, including AMHP, Practice Educator and court skills training, alongside clear career progression pathways. We operate a hybrid working model, with a minimum of two days per week based at Time Square, Warrington, combined with visits to people in their own homes and other community settings.

The role

The Localities Service provides assessment, support planning and care management for adults living with a range of long-term and complex health and social care needs, including older adults living with frailty, dementia, sensory impairment and other significant health conditions.

Working within the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation, you will undertake strengths-based assessments, develop personalised support plans and review ongoing care arrangements to ensure people achieve the outcomes that matter most to them.

Key responsibilities include:

• Undertaking assessments of need and risk, including Mental Capacity Assessments and Best Interest decisions.
• Developing and reviewing person-centred support plans that promote independence, choice and wellbeing.
• Managing and reviewing care packages, including home care, direct payments and 24-hour care arrangements.
• Completing safeguarding enquiries and contributing to Court of Protection work where required.
• Supporting individuals who may be reluctant to engage with services, including people with complex needs and hoarding behaviours.
• Participating in the team's duty rota, responding to referrals and urgent enquiries.
• Working collaboratively with families, carers, health colleagues and partner agencies to achieve positive outcomes.
• Maintaining a robust caseload with the support of regular supervision and a visible, approachable management team.
• As a valued member of the team, you will have opportunities to develop your expertise across a wide range of practice areas, including safeguarding, Continuing Healthcare, risk assessment, court work and complex care planning.

What's needed?

We are looking for enthusiastic and committed social workers who are passionate about making a difference in the lives of adults and older people.

You will:

• Hold a recognised Social Work qualification and be registered with Social Work England.
• Have experience working with adults and/or older people, or be a Newly Qualified Social Worker looking to develop your career.
• Demonstrate a commitment to strengths-based, person-centred practice.
• Have effective communication and interpersonal skills, both written and verbal.
• Be able to work confidently with individuals, families, carers and partner organisations.
• Demonstrate strong organisational and time-management skills.
• Be resilient and able to manage competing priorities in a fast-paced environment.
• Be committed to continuous professional development and high standards of practice.

In relation to this post, we will consider applications from Newly Qualified Social Workers, Social Workers at a Grade 7 or Grade 8 depending on experience.
